We aim to offer you a good service and to help to achieve this we require your assistance in the following ways: 

• Please try to attend surgery whenever possible, visits are only for those patients who are too ill or frail to attend. 
• When possible, make requests for home visits early before 10.00am. 
• Try to treat our staff with courtesy and respect and they will do likewise. 
• Try to keep us informed of your up to date address and telephone number so that we can contact you.
